Covid Booster Shots

Covid-19 booster vaccine for adults and children over the age of 5.

JCC Bridgewater and Green Brook Family Medicine will be providing Pediatric Pfizer vaccines and boosters for ages 5-11.

• Booster is given 5 months after dose #2.
• Pfizer vaccines and boosters for ages 12 and up.
• Moderna vaccines and boosters for ages 18 and up.

Note additional dates may be added on the registration website based on demand.

At the Clinic:

1. Please bring your insurance card to the clinic if you have insurance. The vaccine is free of charge.
2. Please bring in your COVID vaccine card if you are receiving a booster.
3. A parent/guardian is required to be present with children under the age of 18.
4. Dr. Frank will be available at the clinic to address any COVID vaccine related questions.
